This questionnaire asks questions regarding Personal Protective Equipment (PPE) used on board ships. It is part of a survey conducted by researchers at Solent University in collaboration with CHIRP Maritime.

We are asking as many seafarers as possible to fill in the questionnaire. Completing the questionnaire is entirely voluntary and strictly confidential. Your identity and responses to the questionnaire will be completely protected.

The questionnaire should take approximately 15 minutes to complete. Please read each question carefully and mark the response that BEST reflects your opinion.

Your responses are valuable and will be used to help develop ways of providing a safer work environment for seafarers around the world. Your participation is greatly appreciated!

If you have any questions or queries about the study or the questionnaire, please do not hesitate to contact us by e-mail (see below).
